Ischemic Orbital Compartment Syndrome following Spinal Surgery.

Ocular tonometry showed intraocular pressure of 18 mm Hg. Magnetic resonance imag ing (MRI) showed en larged hyperintense extraocular muscles (left superior rectus, inferior rectus and medial rectus) (Figure 1). MRI of brain did not show any abnormalities in optic nerves, occipital lobes and res t o f the v i sua l pa thways and cavernous sinus. Blood investigations including blood biochemistry were normal. Diagnosis of ischemic orbital compartment syndrome was made. High dose IV methylprednisolone (1 gm) was started. Canthotomy and cantholysis was performed but there was no improvement in visual acuity or ocular movements.
